Membership Application

• I hereby apply for membership of The Maze NYC. If accepted, I agree to be bound by the rules of the club. 
• Please note that all sections must be completed in order for this application to be submitted to the committee. Any application missing information will be deemed incomplete and not considered.
• All information shall be treated in confidence.
• A clear and recent headshot must be included. 
• The Maze’s current membership dues and initiation fees are as follows: General Membership dues are $3,300.00 per annum (inclusive of sales tax),
plus a one-time initiation fee of $1,500.00.
• Your credit card will not be charged unless your membership is approved
• Both membership dues and initiation fees are subject to applicable taxes and services fees. 
• This form will take around ten minutes to complete.
