Membership Application

• I hereby apply for membership of The Maze NYC.
If accepted, I agree to be in compliance with the rules of the club.
• Please note that all sections must be completed in order for this application to be submitted to the membership committee.
Any application missing information will be deemed incomplete
and not considered.
• All information shall be treated in confidence.
• A clear and recent headshot must be included. Selfies are acceptable. 
• The Maze’s current membership dues and initiation fees are as follows: General Membership dues are $3,000 per annum,
plus a one-time initiation fee of $500. 
• Both membership dues and initiation fees are subject to applicable taxes and service fees.
• This form will take around ten minutes to complete.
